In short: Massimo Troisi was Italy's beloved comedian of feelings, a master of confused speech and expressive mimicry. Yet, his final masterpiece, Il Postino, was filmed while he suffered from severe heart disease. He famously declined a heart transplant, fearing that a new heart would fundamentally alter the emotions he channeled as an actor.

Born in 1953 near Naples, Massimo Troisi began his career in cabaret, eventually rising to national fame as the leader of the trio La Smorfia. His comedic style, characterized by a unique blend of hesitation and facial nuance, drew inspiration from legendary figures like Totò and the De Filippo brothers. By the 1980s, he had established himself as a formidable writer and director, with films like I'm Starting back from Three exploring the complexities of love and Neapolitan life.

Troisi’s health was compromised early by rheumatic fever, which left him with chronic heart issues. By 1976, he required a valve operation in the United States. Despite his deteriorating condition, he remained deeply committed to his craft. While preparing for his final film, Il Postino: The Postman, he famously postponed necessary surgery to ensure the project was completed. He had previously rejected a scheduled heart transplant, explaining to director Michael Radford that an actor is a man of emotion, and he feared losing his artistic identity with someone else's heart.

His dedication proved fatal. Troisi died of a heart attack in June 1994, just 12 hours after filming concluded. His performance in Il Postino earned him posthumous Academy Award nominations for Best Actor and Best Writing, making him the sixth person in history to receive such an acting nod after death. Eduardo De Filippo, a titan of 20th-century Italian drama, once described him as a comedian of the future, firmly rooted in the past—a legacy that continues to resonate in Italian cinema today.
