Acidosis: How Your Body Balances Blood pH in Seconds and Days
In short: Your body fights acidity with two systems: fast breaths and slow kidneys. Respiratory acidosis happens when lungs can't clear carbon dioxide, while metabolic acidosis stems from kidney failure or excess acid. Both disrupt blood pH, with lungs acting quickly and kidneys compensating over days.
Acidosis is a condition where the blood becomes too acidic, disrupting the body's delicate pH balance. The body maintains blood pH between 7.35 and 7.45 through two primary mechanisms: the lungs and the kidneys. The lungs remove carbon dioxide, which forms carbonic acid when combined with water, while the kidneys regulate bicarbonate levels to neutralize excess hydrogen ions. These systems work in tandem to keep the blood pH stable.
Respiratory acidosis occurs when the lungs fail to remove carbon dioxide efficiently, leading to a buildup of carbonic acid and a drop in blood pH. This can result from chronic lung diseases, neuromuscular disorders, or sedative overuse. Acute respiratory acidosis can cause severe symptoms like confusion and coma, while chronic cases may be asymptomatic due to kidney compensation over days. In contrast, metabolic acidosis arises from issues like kidney failure, severe diarrhea, or excessive acid production, such as from ketoacidosis or lactic acidosis.
The diagnosis of acidosis involves arterial blood gas tests to measure blood pH, carbon dioxide levels, and bicarbonate concentrations. Additional tests like chest X-rays, CT scans, and urine analysis help identify the underlying cause. Treatment focuses on addressing the root cause, such as improving ventilation for respiratory acidosis or managing kidney function for metabolic acidosis. The body's ability to compensate for acidosis varies, with the lungs responding quickly and the kidneys taking days to adjust.
