In short: When your intestines become blocked, the consequences can be life-threatening. From scar tissue following surgery to tumors or twisted loops of bowel, the causes vary, but the warning signs—severe abdominal pain, bloating, and vomiting—require urgent medical evaluation to prevent tissue death, perforation, and systemic infection.

A bowel obstruction occurs when the normal movement of digestive products is halted, either in the small or large intestine. In the United States, the most frequent cause of small bowel obstruction is the formation of abdominal adhesions—bands of scar tissue that develop following previous abdominal or pelvic surgeries. Other common triggers include hernias, where a segment of the intestine becomes trapped, and cancerous tumors. Large bowel obstructions are most commonly driven by colon cancer, though conditions like diverticulitis or volvulus—where the intestine twists—can also be responsible.

The clinical presentation often depends on the location of the blockage. Small bowel obstructions typically manifest as sharp, intermittent waves of cramping pain, often accompanied by nausea and vomiting. In contrast, large bowel obstructions tend to cause more continuous pain and severe constipation. In both cases, the buildup of fluids, gases, and digestive juices creates dangerous pressure. If left untreated, this pressure can lead to perforation, where the intestine tears, or strangulation, where the blood supply is cut off, causing tissue death and gangrene. These complications can trigger peritonitis or life-threatening sepsis.

Diagnosis often relies on imaging, such as CT scans or X-rays, to identify the site and severity of the blockage. While some cases may be managed conservatively with intravenous fluids, pain medication, and nasogastric tubes to decompress the gut, surgery is frequently necessary, particularly for small bowel obstructions. Because symptoms can be nonspecific—sometimes mimicking other conditions or even presenting as metastatic cancer—maintaining a high index of suspicion is critical. Whether the obstruction is partial or complete, the inability to pass gas or stool is a hallmark sign that necessitates immediate emergency care.
