The Department of Defense's only Level 1 trauma center
In short: In the vast landscape of military medicine, one facility stands alone as the pinnacle of emergency care. Brooke Army Medical Center is the only Level 1 trauma center in the entire Department of Defense, serving as the primary hub for the most critical wounded service members and the local community.
The United States Army's leading medical facility, Brooke Army Medical Center (BAMC), sits at Fort Sam Houston. While it serves as a massive 425-bed academic medical center, its role is uniquely significant: it is the only facility within the Department of Defense authorized as a Level 1 trauma center. This means it is equipped to handle the highest acuity of injuries, providing 24-hour emergency services and specialized care for wounded service members and the San Antonio community.
The facility's evolution is a testament to military necessity. It began in 1879 as a small 12-bed wooden dispensary. By 1937, it had expanded to a 418-bed hospital, a transition sparked by Brigadier General Roger Brooke, who pioneered routine chest X-rays in military medicine. During World War II, the hospital's capacity was pushed to extremes, converting barracks into wards to manage an overwhelming flow of casualties from the battlefields.
Modern BAMC is a complex network of ten separate organizations, including community clinics and the Center for the Intrepid for outpatient rehabilitation. In 2011, it absorbed the inpatient services of the Air Force's Wilford Hall Medical Center to create the San Antonio Military Medical Center (SAMMC), though the BAMC designation remains the primary command for the region. It also houses the U.S. The DoD's sole American Burn Association verified Burn Center is the Army Institute of Surgical Research Burn Center.
The facility continues to expand to meet modern demands. A 2009 project added a consolidated tower housing a psychiatric unit, a SICU, and a CCU. While it currently operates with 425 beds, it is designed to expand to 653 beds in the event of a disaster, ensuring it remains the backbone of regional military health.
