Can we teach the human heart to heal itself like an embryo?
In short: The human heart is one of our least regenerative organs, often replacing dead muscle with permanent scar tissue. But as scientists explore the potential of stem cells, a new frontier of regenerative medicine is emerging to move beyond managing symptoms toward true repair.
In the later stages of pregnancy, over one-third of an unborn baby's heart muscle cells multiply to build the organ. However, this regenerative capacity declines within weeks of birth, leaving adult heart muscle cells rarely able to reproduce themselves. When a heart attack occurs due to blocked arteries, the lack of oxygen causes muscle cells to die, which are then replaced by scar tissue. This process can lead to heart failure, a condition where the heart cannot pump blood effectively. In the UK alone, approximately 920,000 people live with heart failure.
To combat this, researchers are investigating various stem cell types. Cardiac stem cells found within the heart can differentiate into cardiomyocytes—the cells responsible for heart contractions—and promote angiogenesis, the formation of new blood vessels. Researchers are also investigating Induced Pluripotent Stem Cells (iPSCs); by reverting adult cells to an embryonic-like state, these allow for personalised treatments with lower rejection chances. While embryonic stem cells are highly versatile, they face ethical concerns and risks of tumor formation.
Innovative techniques are currently in development. At the University of Cambridge, Professor Sanjay Sinha’s team is growing patches of real heart tissue in a dish using stem cells and growth factors on collagen scaffolds. Other researchers, such as Dr. Mairi Brittan at the University of Edinburgh, are studying endothelial progenitor cells to encourage new blood vessel growth. While clinical trials are underway to test these therapies, the goal remains to move from traditional treatments like medication and surgery toward true myocardial repair.
