In short: Centanafadine, recently approved for ADHD, breaks from traditional stimulants by using a rigid, bicyclic structure. Unlike older drugs that force neurotransmitters out of neurons, this triple reuptake inhibitor blocks the reabsorption of norepinephrine, dopamine, and serotonin. It aims to provide effective symptom management while potentially reducing common stimulant-related risks.

Approved by the FDA on July 24, 2026, centanafadine—marketed as Simtriyo—represents a shift in medicinal chemistry for ADHD treatment. Traditional stimulants like amphetamines are flexible molecules that enter neurons and trigger the release of neurotransmitters. In contrast, centanafadine features a rigid azabicyclo[3.1.0]hexane core and a 2-naphthyl ring, designed to lock the molecule into a specific shape that binds to transporters without causing neurotransmitter efflux.

The drug acts as a triple reuptake inhibitor, prioritizing norepinephrine, followed by dopamine and serotonin. Cryo-EM analysis shows the molecule fits deeply into the dopamine transporter's central binding pocket, where its structure creates a physical barrier that prevents dopamine reuptake. This binding is stabilized by a salt bridge between the molecule's nitrogen atom and the D79 residue, a mechanism that is conserved across the norepinephrine and serotonin transporters as well.

Development of this compound was a multi-decade process, evolving from earlier research into phenethylamine-based inhibitors like bicifadine and amitifadine. Otsuka Pharmaceutical acquired the rights to the compound in 2017 after it was spun off from Euthymics Bioscience. While it offers a novel pharmacological profile, it carries warnings regarding potential abuse and suicidal ideation, and clinical trials have noted side effects such as headache, appetite loss, and insomnia.

The drug is currently indicated for adults and children aged six and older who weigh at least 20 kg. Because it is primarily metabolized by monoamine oxidase A, it cannot be used alongside monoamine oxidase inhibitors. Ongoing research is exploring its potential utility for other conditions, including anxiety disorders and smoking withdrawal, as scientists continue to refine how these rigid molecular scaffolds can modulate central nervous system function.
