Cranial nerves control everything from vision to swallowing—what happens when they fail?

Cranial Nerves: Basic Anatomy, Functions, Effects of Damage, and Clinical Tests, Animation · 9:36 · Video: Alila Medical Media · Watch at the source

In short: Cranial nerves govern vision, taste, swallowing, and more. Damage to these 12 nerves can lead to blindness, double vision, or even life-threatening conditions. Understanding their functions and how to test for damage is key to diagnosing neurological disorders.

Cranial nerves are the 12 pairs of nerves that originate from the brain and extend to various parts of the head, face, and neck. Each nerve has a specific function, ranging from sensory input to motor control and parasympathetic innervation. The optic nerve, which is cranial nerve II, plays a key role in vision, and harm to this structure may cause partial or complete loss of sight. To evaluate potential nerve injury, vision clarity is checked, focusing on each eye separately to pinpoint the affected pathway.

The oculomotor nerve (CN III) controls most eye movements, eyelid opening, and pupil constriction. It originates in the midbrain and contains both somatic and parasympathetic fibers. Damage to CN III can lead to a characteristic 'down and out' deviation of the eye, along with drooping eyelid, dilated pupil, and loss of accommodation reflex. Clinical tests include assessing pupillary response to light and the ability to track moving objects.

Other cranial nerves, such as the trigeminal nerve (CN V), have multiple divisions and serve both sensory and motor functions. The facial nerve (CN VII) is a mixed nerve that controls facial expressions, taste, and salivary and tear production. The vagus nerve (CN X) is a major parasympathetic nerve that regulates vital functions such as heart rate, digestion, and breathing. Damage to these nerves can lead to severe complications, including difficulty swallowing or speaking, and even life-threatening conditions like respiratory failure.
