In short: Medicaid is the largest source of funding for low-income health services in the United States, yet measuring its reach remains a complex data challenge. While researchers work to standardize how we track home- and community-based services, inconsistent reporting across states reveals significant gaps in our understanding of this vital safety net.

Established by the Social Security Amendments of 1965, Medicaid provides essential health insurance for millions of low-income and disabled Americans. By 2022, the program covered 85 million people, serving as a critical pillar for long-term care, including nursing homes and home- and community-based services (HCBS). Despite its scale, the program’s decentralized nature—managed by states with federal oversight—creates substantial hurdles for researchers attempting to analyze service utilization through claims data.

A recent evaluation of Medicaid Statistical Information System Analytic Files (TAF) highlights the difficulty of achieving accurate, nationwide measurement. When researchers compared existing algorithms to identify HCBS users, they found that while results were generally concordant, specific states produced implausible figures. For instance, Pennsylvania and Illinois reported unexpectedly high counts, while Mississippi showed unusually low numbers. These discrepancies often stem from missing program codes, varied service classifications, and inconsistent handling of capitated payments.

The complexity is further compounded by reporting volatility. In several states, HCBS user counts fluctuated by more than 50% between 2021 and 2023, suggesting that data quality or coding practices are shifting over time. While new, unified algorithms aim to resolve these discordances by incorporating waiver identification numbers and refining data elements, the underlying reliability of state-level reporting remains a persistent limitation. Without standardized data, policymakers struggle to gain a clear, accurate picture of how these essential services are being delivered across the country.
