In short: If you feel dizzy when rolling over in bed or tilting your head, you may have benign paroxysmal positional vertigo. This condition occurs when tiny calcium crystals in your inner ear dislodge and drift into the wrong chamber, sending false motion signals to your brain that trigger intense, spinning sensations.

Benign paroxysmal positional vertigo, or BPPV, is a mechanical disorder of the inner ear. Your utricle, a chamber responsible for detecting horizontal motion, normally houses calcium carbonate crystals known as otoconia or ear rocks. When these crystals become dislodged due to factors like age-related wear, head trauma, migraines, or infections like labyrinthitis, they can migrate into the fluid-filled semicircular canals.

These canals contain cilia, tiny hair-like structures that detect fluid movement to inform your brain about your head's position. When loose crystals roll through these canals, they displace the fluid, creating a false signal of motion. Your brain then compares this ear-derived data with input from your eyes and body. Because the signals conflict, you experience vertigo—a sudden, often intense sensation that the world is spinning.

Symptoms typically last less than a minute and are triggered by specific movements, such as looking upward or turning over while sleeping. Beyond the spinning sensation, you might experience nausea, lightheadedness, or nystagmus, which is an involuntary, rapid movement of the eyes. While BPPV is not inherently dangerous, the resulting disorientation can lead to falls or other accidents, significantly impacting your quality of life if left unaddressed.

The condition is often manageable, but because episodes can be upsetting and recurrent, professional diagnosis is important. It is essential to distinguish BPPV from other neurological issues, as the presence of symptoms like numbness or weakness would suggest a more serious underlying cause. By understanding that your vertigo is a physical displacement of crystals rather than a systemic illness, you can better navigate treatment options to stop the room from spinning.
