In short: Before it became a beloved Showtime staple, Nurse Jackie was a darker, supernatural-tinged project titled Nurse Mona. It eventually evolved into a sharp, medical comedy-drama that used the absurd rationalizations of addiction to fuel its storytelling, earning massive critical acclaim and multiple Emmy nominations.

Conceived by Cary producer Caryn Mandabrag, the series was inspired by real-life stories from an ER nurse. While the original concept, 'Nurse Mona,' featured vague supernatural elements, the show was eventually reworked by creators Liz Brixius and Linda Wallem. They infused the script with comedy, focusing on the 'absurd' way addicts rationalize their behavior to underplay the damage they cause.

The show's authenticity was deeply personal; lead actress Edie Falco and showrunners Brixius and Wallem were all recovering alcoholics. This shared experience informed the portrayal of Jackie Peyton, a New York City nurse struggling with dependencies on Vicodin, Percocet, and Xanax. The series balanced sharp comedy with the heavy ramifications of addiction, particularly during the fourth season when Jackie entered rehab.

Despite its success—including 24 Emmy nominations and wins for both Falco and Merritt Wever—the show faced criticism. The New York State Nurses Association argued that Jackie's ethical violations promoted a negative image of the profession. The series concluded in June 2015 with an ambiguous finale involving a heroin overdose, leaving viewers to debate the ultimate fate of the character.

The production underwent significant shifts, including a change in showrunners after Brixius and Wallem departed due to creative disagreements. New showrunner Clyde Phillips was brought in to make the series even darker and more focused on the 'shrapnel'—the collateral damage Jackie's 'triple life' caused to her family and colleagues at All Saints' Hospital.
