In short: The endometrium is more than just a lining; it is a dynamic tissue capable of regenerating, responding to complex hormonal signals, and even forming a placenta. However, when this process goes awry, it can lead to chronic inflammatory conditions that affect millions of people worldwide.

The endometrium is the inner lining of the uterus in mammals, consisting of a single layer of columnar epithelium and a connective tissue layer called the stroma. It is organized into two distinct layers: the basal layer and the functional layer. The basal layer contains stem cells that remain intact throughout the menstrual cycle to regenerate the functional layer, which thickens and is shed during menstruation in humans.

S2:p1, S2:p2, S2:p4 The tissue is highly responsive to hormones. Estrogen induces proliferation during the follicular phase, while progesterone drives changes during the luteal phase. In the absence of progesterone, the blood vessels supplying the functional layer constrict, causing the cells to die and shed. This process is so specialized that about 70% of the 20,000 protein-coding genes expressed in human cells are active in the normal endometrium. S2:p3, S2:p5, S2:p7

When this tissue grows outside the uterus, it is known as endometriosis. This chronic inflammatory condition affects approximately 10% of reproductive-age women. It can occur in the pelvis, ovaries, fallopian tubes, or even outside the pelvis in areas like the chest. Because symptoms like severe period pain, fatigue, and pelvic pain can mimic other conditions like irritable bowel syndrome or fibroids, diagnosis can be lengthy and often requires multiple tests, including ultrasounds, MRIs, or a laparoscopy.

S3:p1, S1:p2, S1:p3, S1:p4, S1:p5 Research into the condition faces significant hurdles due to sample heterogeneity. For example, studies on the MAP4K4 protein—a regulator of cell motility—have shown inconsistent results across different datasets, highlighting the need for more rigorous histological validation and spatially resolved analysis to understand how the disease progresses.
