In short: Fluoride is a staple of modern dental hygiene, but the line between prevention and poisoning is governed by precise concentrations. While it protects teeth at low levels, excessive exposure can lead to systemic issues, ranging from gastrointestinal distress in children to severe bone deformities in contaminated regions.

Acute fluoride overdose is relatively rare and typically involves children accidentally ingesting large amounts of household products. Common sources include toothpaste, mouthwashes, and vitamins like Tri-Vi-Flor. However, more severe cases can involve industrial chemicals such as hydrofluoric acid found in rust removers. In these instances, medical professionals may monitor vital signs and perform tests like blood work, urine analysis, and electrocardiograms (ECG). Treatment can involve IV fluids, medications, and even mechanical ventilation if breathing is compromised.

The severity of an overdose often depends on the source and speed of medical intervention. While the fluoride in toothpaste is usually insufficient to cause harm, children may experience gastrointestinal distress from flavored products. During the period 1990–1994, more than 628 individuals, primarily minors, received medical care for consuming fluoride from toothpaste. In contrast, industrial exposure or the consumption of sodium fluoride (NaF) in high doses can be lethal; the estimated lethal dose for most adults is 5 to 10 grams of sodium fluoride.

On a broader scale, environmental contamination presents a different risk. In India, an estimated 60 million people have been affected by well water contaminated by naturally occurring fluoride leached from granite rocks, leading to significant bone deformities in children. To mitigate these risks, the U.S. A limit of 0.7 milligrams of fluoride per litre in water is advised by the Health and Human Services Department. This highlights the importance of concentration: while fluoride is a tool for health, its toxicity is a matter of strict dosage and environmental management.
