Not all blood cancers are created equal: the lymphoma divide

In short: While leukemia and lymphoma both involve white blood cells, they differ in where they live and how they behave. Understanding the distinction between Hodgkin and non-Hodgkin lymphoma reveals how specific cellular markers—like a single unique cell type—can completely change a medical diagnosis.

Lymphoma is a group of cancers that develop within the lymphatic system, including lymph nodes, the spleen, and lymphoid tissues. It primarily affects mature lymphocytes: B-cells, T-cells, and natural killer cells. These cells are produced in the bone marrow and mature in primary lymphoid organs like the thymus before migrating to secondary sites to fight pathogens. Because the bone marrow serves as both a blood production site and a lymphoid organ, cancers arising from B-cells there can be classified as both leukemia and lymphoma.

The distinction between the two main types of lymphoma—Hodgkin and non-Hodgkin—is primarily histological. If a microscope reveals the presence of a specific large cell called a Reed-Sternberg cell, the cancer is classified as Hodgkin lymphoma. If these cells are absent, it is non-Hodgkin. While Hodgkin lymphoma typically arises from B-cells and shows a bimodal age distribution (peaking in the 20s and again in the 70s and 80s), non-Hodgkin lymphoma is much more common and can originate from B-cells, T-cells, or natural killer cells.

Clinical behavior varies significantly between subtypes. Non-Hodgkin lymphomas are generally more prevalent in older adults, though certain types like Burkitt’s lymphoma occur frequently in children. Some non-Hodgkin varieties may behave like chronic conditions, relapsing over many years, and are often associated with a worse prognosis than Hodgkin lymphoma. Diagnosis relies on physical exams for swollen nodes and 'gold standard' tissue biopsies, while treatment ranges from 'watch and wait' for low-grade cases to advanced interventions like CAR T-cell therapy.
