Can we store flu medicine longer? New research suggests a simple solution for pandemics.
In short: Oseltamivir is a cornerstone of pandemic preparedness, but its shelf life in liquid form has historically been limited. A recent study reveals that a specific 15 mg/mL oral solution remains stable for up to 85 days when frozen and then refrigerated, potentially simplifying large-scale distribution during future public health crises.
Oseltamivir, widely known by the brand name Tamiflu, is a neuraminidase inhibitor used to treat and prevent influenza A and B. Since its approval in 1999, it has become a staple of global health, though its role remains a subject of intense debate. While major organizations like the CDC and the WHO recommend it for high-risk groups and critically ill hospitalized patients, other researchers—notably in a 2014 Cochrane Review—have questioned its efficacy in reducing hospitalizations or serious complications. This controversy highlights the complexity of balancing clinical guidelines with evolving evidence.
Beyond clinical efficacy, logistical challenges like storage are critical for pandemic readiness. A recent study tested the stability of an extemporaneously prepared 15 mg/mL oral solution stored in amber glass bottles. Researchers found that by freezing the solution at -20 °C for up to 75 days, followed by 10 days of refrigeration after thawing, the drug maintained its chemical and physical integrity. This 85-day window offers a practical strategy for managing large volumes of medication when traditional supply chains might be strained.
However, the drug's long-term utility faces biological hurdles. Recent surveillance data from 2023 to 2026 identified the emergence of A(H1N1)pdm09 influenza viruses carrying double mutations, specifically NA-I223V and NA-S247N. These variants, observed in countries including the Netherlands, Spain, Finland, and France, demonstrated a 12- to 13-fold reduction in susceptibility to oseltamivir. Such findings emphasize that while pharmaceutical stability is achievable through careful storage, the ongoing evolution of the influenza virus requires constant genomic monitoring to ensure that stockpiled treatments remain effective against circulating strains.
