In short: Sand flies are masters of stealth. These tiny, fragile insects are expanding their range across Europe, fueled by global warming and shifting human behaviors. Because they are non-aquatic and hard to target with traditional insecticides, they are creating new frontiers for diseases like leishmaniasis and severe neuro-invasive viruses.

Phlebotomine sand flies are primary vectors for Leishmania parasites and phleboviruses. While many phleboviruses cause a mild 'three-day fever,' the Toscana virus (TOSV) can lead to severe, sometimes fatal, neuro-invasive disease. In Mediterranean regions, these flies are responsible for zoonotic and anthroponotic leishmaniasis, which can manifest as visceral or cutaneous infections. Notably, while the public health impact is currently highest on the southern and eastern shores of the Mediterranean, these pathogens are becoming an emerging concern in broader Europe.

The expansion of these vectors is driven by a combination of climate change and societal shifts. Various types, such as P. ariasi, P. mascittii, P. neglectus, P. perfiliewi, plus P. perniciosus, are migrating to elevated regions and northerner zones. Furthermore, the movement of infected dogs can establish new infection foci in northern areas where the vector is already present. Interestingly, these flies have shown remarkable ecological plasticity, adapting to modern peri-urban landscapes—including residential gardens and high-density apartment blocks—where they feed on domestic reservoirs like dogs.

Control remains difficult because sand flies are terrestrial breeders and are not susceptible to large-scale insecticidal programs. Unlike some other vectors, they are too fragile to be moved passively over long distances, meaning their spread is primarily driven by local environmental changes. Because Europe currently lacks a region-wide surveillance scheme for these pathogens, and human leishmaniasis is not yet included in EU surveillance lists, the insects' northward migration poses a significant, under-monitored public health challenge.
