In short: In 1780, a group of surgeons met in the Elephant public house to change medical history. What began as a local effort to separate surgeons from barbers has evolved into a world-ranked university with a global footprint.

The roots of the Royal College of Surgeons in Ireland (RCSI) lie in the medieval practice of surgery in Dublin, which was originally licensed by the Barber-Surgeons' Guild. This guild, also known as the Guild of St. Mary Magdalene, had been incorporated by royal decree of Henry VI in 1446, making it the first medical corporation in Britain or Ireland. For centuries, surgeons followed a strict path of a three-year apprenticeship and two years as a journeyman, a mandatory requirement that lasted until 1828.

The modern institution emerged from a desire for standardized education. In 1765, surgeon Sylvester O'Halloran proposed a college modeled after the College de St. Cosme in Paris. This led to the founding of the Dublin Society of Surgeons in 1780 at the Elephant public house. Following a petition to the Lord Lieutenant in 1781, King George III granted a Royal Charter on 11 February 1784, officially establishing the College. The first meeting of its governing body, led by President Samuel Croker-King, took place on 2 March 1784, at the Rotunda Hospital.

The College's history is marked by significant social and academic shifts. It was a pioneer in inclusivity, becoming the first medical school in Ireland to admit women in 1885, with Emily Winifred Dickson becoming the first female Fellow in 1893. While its curriculum once included subjects like botany and military surgery, it has transitioned into a full university, authorized in December 2019. Today, it is recognized for excellence, ranking first globally for SDG3 Good Health and Well-being in the 2025 Times Higher Education Impact Rankings.
