The hidden balance of your gut determines your immunity
In short: Most people carry Clostridioides difficile in their intestines without ever feeling a thing. It only becomes a life-threatening threat when the delicate ecosystem of your microbiome is disrupted, allowing this bacterium to seize control and produce toxins that can lead to severe inflammation or even sepsis.
Clostridioides difficile (C. diff) is a bacterium naturally found in the intestines of many healthy individuals; it is estimated to be present in up to 3% of adults and as many as 66% of babies without causing any symptoms. Under normal conditions, it lives harmlessly alongside a diverse community of other bacteria. However, when this balance is shifted—most commonly by the use of antibiotics—the protective 'good' bacteria are disadvantaged. This allows C. diff to grow to unusually high levels and produce toxins that attack the intestines.
The infection is particularly prevalent in healthcare settings like hospitals and nursing homes, where it remains the leading cause of infectious diarrhea in hospitalized patients. Risk factors include being over 65 years old, having a weakened immune system (due to conditions like diabetes, kidney failure, or chemotherapy), or taking medications that reduce stomach acid, such as proton pump inhibitors. Notably, C. diff can exist outside the body as spores—inactive forms with protective coatings that allow them to survive on surfaces and in soil for months or even years.
Once these spores are swallowed and reach the intestines, they become active again. While most healthy people resist infection, those with compromised systems may suffer from symptoms ranging from fever and stomach pain to serious complications like colitis or toxic megacolon. Treatment typically involves stopping current antibiotics and starting a new 10-day course of specific medication. In persistent cases where the infection returns two or more times, patients may be offered a faecal microbiota transplant to restore their gut's natural bacterial balance.
