A heart surgeon turned president, Masoud Pezeshkian navigates Iran's complex political and social landscape.
In short: Masoud Pezeshkian, a cardiac surgeon and former health minister, rose to the Iranian presidency in 2024. Known for his reformist leanings and ethnic background, he balances a desire for international engagement with the constraints of a conservative establishment, all while navigating the volatile aftermath of regional conflicts and domestic unrest.
Born in 1954 in Mahabad, Pezeshkian’s identity is rooted in his diverse heritage, with an Azerbaijani father and a Kurdish mother. His early life was marked by the 1979 revolution and the subsequent Iran-Iraq War, where he served as both a combatant and a medic. This period of service preceded his medical career, during which he specialized in heart surgery and eventually led the Tabriz University of Medical Sciences. A personal tragedy struck in 1993 when a car accident claimed the lives of his wife and youngest son, leaving him to raise his remaining three children alone.
Pezeshkian’s political trajectory began in the administration of reformist president Mohammad Khatami, where he served as deputy health minister and later as minister of health from 2001 to 2005. Despite his reformist label, he describes himself as a 'reformist principlist,' aiming to improve the Islamic Republic from within. His tenure in the Majles, spanning 2008 to 2024, saw him emerge as a vocal critic of government crackdowns on protests and the strict enforcement of hijab mandates, particularly following the death of Jina Mahsa Amini in 2022.
Since assuming the presidency in 2024, Pezeshkian has faced immense challenges, including the death of Supreme Leader Ali Khamenei in early 2026 and the subsequent regional instability. While he initially sought to revive nuclear agreements with the West, his administration has been largely excluded from major security and military strategy by the Islamic Revolutionary Guard Corps. Today, his government focuses primarily on domestic economic issues, operating within a restricted political environment that continues to shape the future of the Iranian state.
