How a Court Jester Founded England’s Oldest Hospital Nine Centuries Ago
In short: St Bartholomew’s Hospital, known as Barts, has provided continuous care in London since 1123. Founded by a courtier named Rahere after a life-changing illness, the institution has survived for 900 years. Its history reveals a remarkable evolution from a medieval religious foundation to a pioneer of modern medical science.
The story of Barts begins with Rahere, an ambitious figure in the court of Henry I, often described as a jester. During a pilgrimage to Rome, Rahere contracted a severe illness, likely malaria. Fearing for his life, he vowed to establish a hospital for the poor. Upon returning to England, he claimed a vision of St Bartholomew directed him to build at Smithfield, a marshy site in the London suburbs then used for executions.
Construction took about 10 years, and Rahere led the hospital for two decades until his death in c. 1143. In its early years, the hospital was managed by eight brethren and sisters of the priory. Care was primarily focused on rest, food, and prayer, with many reported cures attributed to St Bartholomew rather than clinical intervention. However, the hospital gradually gained autonomy, securing its own seal by 1200 and achieving formal papal confirmation of its independence in 1453.
The hospital’s longevity is documented in an extensive archive containing over 15,000 items, including a grant from 1137 that mentions Rahere. Over the centuries, Barts became a crucible for medical advancement. Notable figures include Sir James Paget, who identified bone and breast diseases, and Ethel Gordon Fenwick, who campaigned for state nursing registration. Today, Barts remains the oldest hospital in England operating on its original site, serving as a testament to centuries of institutional adaptability and vision.
