The United States has the highest maternal death rate among developed nations

In short: Despite being a leading healthcare power, the U.S. faces a crisis where 60 percent of new mother deaths are preventable. Addressing this requires moving beyond reactive fixes to a life-course approach that treats pregnancy as an opportunity to improve long-term health for both mother and child.

The disparity in maternal outcomes is stark. While England saw significant reductions in stillbirths (20%) and neonatal deaths (30%) between 2010 and 2021, other quality indicators have declined. Furthermore, systemic inequities persist: black and Asian women, as well as those from more deprived areas in the UK, experience significantly worse outcomes than other demographics.

Experts suggest shifting the focus from merely investigating failures to identifying 'positive deviance.' Rather than only asking what went wrong, this framework asks what specific behaviors, processes, and systems contribute to safe care. This is complemented by a need for four critical pillars in maternity services: kind and compassionate care, teamwork with common purpose, the capacity to identify poor performance, and organizational oversight.

A 'life-course approach' is central to modern strategy. Because adverse perinatal experiences have profound inter-generational effects on maternal and infant health, pregnancy serves as an optimal window for preventative measures that extend beyond childbirth. The goal is to translate high-quality evidence into frontline primary and secondary care to improve general health outcomes regardless of a woman's socioeconomic situation.

The challenge lies in implementation and scale. While the evidence for what works exists, successfully integrating it into universal public services requires complex service delivery science. This involves linking clinical trials and data modeling with qualitative research—such as interviews and ethnography—to ensure that improvements reach multicultural and inner-city populations effectively.
