The most dangerous day of a child's life is their first

Suffer the Little Children: How to Revolutionise Neonatal Health - Professor Chris Whitty · 43:38 · Video: Gresham College · Watch at the source

In short: While childhood survival has reached historic highs, a grim paradox has emerged: as we successfully protect older children, newborns are becoming the primary victims of global mortality. We are approaching a tipping point where nearly half of all deaths in children under five occur within just 28 days of birth.

Despite dramatic progress in global health over the last two decades, neonatal mortality—deaths occurring in the first 28 days of life—is declining at a significantly slower rate than deaths in older children. This disparity means that while childhood is safer than ever before, the proportion of child deaths occurring in the neonatal period is steadily increasing. In many parts of the world, this trend is stark: more than half of all deaths for children under five occur within the first month of life in much of Asia, and about a third occur in that timeframe across Africa.

The risk profile for infants is concentrated in an incredibly narrow window. Most neonatal deaths happen within the first week of life, with the highest proportion occurring in the first 24 hours after birth. Even in high-income nations like the UK, where neonatal mortality rates have fallen by more than half over the last 20 years, the data shows that most children who die before age 20 do so within their first year, specifically on the day of birth.

The disparity between wealthy and developing nations remains a critical hurdle. For instance, while Japan reports a neonatal mortality rate of 1.0 per 1,000 live births, countries like Pakistan report rates as high as 42. Professor Chris Whitty argues that these deaths are largely avoidable. Crucially, the most effective interventions to bridge this gap are not complex medical miracles but simple, cheap measures that can be implemented even in low-intensity settings.
