From Ancient Cauterization to Engineering the Immune System

The Evolution of Cancer Therapy · 49:52 · Video: Gresham College · Watch at the source

In short: For millennia, cancer was a mystery of 'black bile' and physical tumors. Today, it is a battle of molecular evolution. We have moved from simply cutting out visible growths to sophisticated therapies that reprogram our own biology to identify and destroy malignant cells at a genetic level.

The history of cancer treatment is a transition from macroscopic observation to microscopic manipulation. In ancient Egypt, as early as 3000 BCE, the Edwin Smith papyrus describes treating tumors through cauterization. By 400 BCE, the Greek physician Hippocrates coined the term 'karkinos' (crab) to describe the way tumors metastasize. However, for centuries, the prevailing theory—proposed by Galen in the 2nd century CE—attributed cancer to an excess of 'black bile,' a belief that stalled therapeutic progress for nearly two thousand years.

The 18th and 19th centuries marked a paradigm shift toward anatomical pathology. In 1761, Giovanni Battista Morgagni began linking cancer to specific organ changes. This was followed by Rudolf Virchow’s groundbreaking work in the 1850s and 1860s, which established cell theory. Virchow proved that tumors are composed of cells derived from healthy tissue, effectively dismantling ancient theories and grounding cancer in biology. This era also saw the birth of modern surgery, enabled by the introduction of anesthesia in 1846 and antisepsis in 1867.

Modern breakthroughs arrived with the discovery of X-rays in 1895 and radioactivity in the late 1890s by Marie Curie and Henri Becquerel, laying the groundwork for radiation therapy. By 1915, researchers identified carcinogens like coal tar. Today, the focus has shifted to clonal evolution and immunotherapy. Current clinical trials aim to train the body's internal defenses to recognize and eliminate disease, moving beyond external intervention toward internal biological orchestration.
