In short: Patented in 1962, temazepam is a benzodiazepine designed to treat severe insomnia. While it effectively reduces sleep latency and increases total sleep time, it is notably more toxic than its peers. It is one of the few benzodiazepines capable of causing fatal overdose even when taken entirely on its own.

Temazepam functions by enhancing the effects of gamma-aminobutyric acid (GABA) in the brain, inducing sedation and muscle relaxation. While the American Academy of Sleep Medicine noted in 2017 that it can reduce sleep latency by 37 minutes and increase total sleep time by 99 minutes, its use is generally restricted to short-term treatment. Guidelines in the UK suggest limiting prescriptions to two to four weeks to avoid the rapid development of tolerance and physical dependence.

The drug is distinguished by its high toxicity profile. Unlike many other benzodiazepines, temazepam has been documented as fatal in overdose cases without the presence of other central nervous system depressants. Research from the 1990s in Britain identified it as having the highest number of deaths per million prescriptions among common medications of the era. Furthermore, studies in Australia found that temazepam overdoses were significantly more likely to result in coma compared to other drugs in the same class.

Beyond its toxicity, temazepam presents significant risks for daily functioning. It can cause cognitive and motor impairment, including ataxia and memory issues, which may persist into the next day. Because it is highly addictive and prone to abuse, the FDA mandated updated boxed warnings in 2020 regarding these dangers. Despite these risks, it remains a tool for specific scenarios, such as the U.S. Air Force using it as a 'no-go pill' to support mission readiness for aviators, provided they undergo ground testing and adhere to strict recovery periods.
