Why a glass of wine might turn your face red and cause immediate discomfort
In short: If you experience facial flushing or a stuffy nose after just one drink, you may have alcohol intolerance. This genetic condition prevents your body from efficiently breaking down alcohol toxins. While often confused with an allergy, it is actually a metabolic disorder that makes consuming alcohol inherently unpleasant.
Alcohol intolerance is an inherited metabolic disorder where the body lacks the specific enzymes required to process alcohol effectively. At its core, the condition involves a genetic mutation that impairs the function of aldehyde dehydrogenase, the enzyme responsible for breaking down acetaldehyde—a toxic byproduct created during alcohol metabolism. When this enzyme is deficient or misfolded, acetaldehyde accumulates in the body, triggering immediate, uncomfortable reactions such as facial flushing, nausea, and nasal congestion.
While this genetic trait is most frequently observed in individuals of East Asian descent, it can affect anyone. The condition is distinct from an alcohol allergy, which is an immune system overreaction to specific ingredients like sulfites, grains, or preservatives found in beverages. Unlike an allergy, which can involve severe symptoms like hives or swelling, alcohol intolerance is primarily a digestive system failure to metabolize ethanol. It does not mean an individual becomes intoxicated faster, but the physical symptoms often lead people to drink less.
Beyond genetics, research indicates that epigenetic factors—such as DNA methylation—can further influence the severity of the condition. These heritable changes in gene expression can reduce enzyme production, effectively intensifying symptoms over time. In rare instances, sudden onset of these reactions later in life may signal underlying health issues, such as Hodgkin lymphoma. For most, however, the only way to manage the condition is to avoid alcohol or identify and steer clear of the specific ingredients that trigger a reaction.
