The carotid bifurcation: a critical crossroads for your brain and face

Carotid Arteries - Internal Carotid Artery (ICA), External Carotid Artery (ECA) - Anatomy of Neck · 18:43 · Video: Medicosis Perfectionalis · Watch at the source

In short: Deep within your neck, two major arteries serve as the primary highways for blood flow to your head. At a specific junction called the carotid bifurcation, they split into distinct paths—one feeding your facial features and the other navigating a complex labyrinth inside your skull.

The human body utilizes two common carotid arteries (CCA) that originate in the upper chest just below the neck. As these vessels travel upward toward the skull, they reach a pivotal anatomical landmark where each one divides into its internal and external branches. This bifurcation is essential for distributing blood to different functional regions of the head. The external carotid artery (ECA) serves the external structures.

It travels along the side of the neck toward the ear before dividing into terminal branches, specifically the maxillary and superficial temporal arteries. In total, the ECA gives rise to eight distinct branches—including the facial, lingual, and occipital arteries—which provide blood to various tissues in the face and neck. In contrast, the internal carotid artery (ICA) is responsible for supplying the brain.

Its journey is more complex; it travels through the neck as the cervical segment (C1) before entering a specific opening in the skull called the carotid canal. This canal marks the boundary between extracranial and intracranial anatomy. Once inside the skull, the ICA follows a winding path divided into six segments, ranging from the petrous to the communicating segments. Understanding this division is vital for medical professionals because it separates the blood supply of the face from the critical circulation of the brain. The transition through the carotid canal represents a significant anatomical shift where the artery must navigate the intricate structures of the cranium.
