The hidden autoimmune response to a common kitchen staple
In short: For many, gluten is just a protein in bread. But for those with coeliac disease, it triggers a destructive autoimmune reaction. This condition goes far beyond a simple stomach ache, potentially causing systemic issues like nerve damage, infertility, and severe malnutrition if left unmanaged.
Coeliac disease is an autoimmune condition where the ingestion of gluten—a protein found in certain grains—causes the body to attack its own small intestine. This damage prevents the bowel from absorbing essential nutrients from food. Because the damage happens at a cellular level, the symptoms can vary wildly between individuals. Some people experience severe gastrointestinal distress, such as chronic diarrhoea or steatorrhoea (stools with high fat content that are greasy and foul-smelling), while others may have no outward symptoms at all.
The systemic impact of malabsorption can be profound. Beyond digestive issues like bloating, constipation, and abdominal pain, the condition can manifest as extreme fatigue, unexpected weight loss, and even peripheral neuropathy, which causes tingling or numbness in the hands and feet. In children, the lack of nutrients can lead to stunted growth in height and weight, delayed puberty, and iron deficiency anaemia. A specific skin manifestation, dermatitis herpetiformis, is estimated to affect about 1 in 4 people with the disease; this itchy, blistering rash typically appears on the elbows, knees, or buttocks.
Diagnosis and treatment are critical because complications can occur even when symptoms are mild or non-existent. The primary treatment is a strict gluten-free diet, which allows the intestinal lining to recover and the rash to clear. Because it is an autoimmune response, the condition is often detected during testing for other health issues, highlighting the importance of early screening for those with a family history or persistent unexplained symptoms.
