The tiny, tube-shaped engine driving your most vital involuntary life functions

In short: Measuring only about 3 centimeters long, the medulla oblongata is a minuscule portion of your brain. Yet, this small cluster of neurons acts as your body's automatic control center, managing the essential rhythms of breathing, heartbeat, and blood pressure that keep you alive.

Located at the very base of the brainstem, the medulla oblongata (also known as the myelencephalon) serves as the critical junction between the spinal cord and the rest of the brain. Via the foramen magnum, located at the skull's base, it joins the spinal cord. Anatomically, it sits below the pons and acts as a primary communication pathway, carrying nearly all nerve pathways between the cerebrum, cerebellum, and the body.

The medulla is a complex hub of integration. It houses the origins of most cranial nerves—excluding those for vision and smell—which coordinate essential functions like swallowing, speech, facial expression, and taste. Internally, it contains specialized groups of nerve cells that regulate the autonomic nervous and vasomotor functions. By regulating blood vessel width to stabilise blood pressure, these hubs synchronise parasympathetic 'rest-and-digest' and sympathetic 'fight-or-flight' activities.

Despite its importance, the structure is remarkably small, weighing only between 59 and 72 grams—roughly 0.5% of the average adult brain. Because so many vital neural structures are concentrated in this tiny space, even minor lesions from trauma, tumors, or strokes can be devastating. Such injuries may lead to respiratory disturbance, speech disorders, or even locked-in syndrome, a rare state where a person remains conscious but loses most voluntary muscle movement. In extreme cases, the loss of brainstem function is considered by some experts to be equivalent to brain death.
