In short: A simple movement or a structural quirk can trigger a cascade of neurological and vascular symptoms. Thoracic Outlet Syndrome occurs when nerves or vessels are compressed in the narrow space between your collarbone and the top of your ribs, potentially causing everything from tingling fingers to life-threatening blood clots.

Thoracic Outlet Syndrome (TOS) is not a single disease but a group of conditions caused by compression in the thoracic outlet—the ring-like area formed by the top ribs and the collarbone. This pressure can stem from anatomical abnormalities, such as a cervical rib (an extra rib growing from the neck spine), which affects between 1% and 3% of the population. Other triggers include poor posture, muscle hypertrophy from bodybuilding, or injuries like whiplash.

The syndrome manifests in three distinct types based on what is being compressed. Neurogenic TOS is the most frequent, accounting for over 90% of cases. In this form, the brachial plexus—the nerve network controlling arm movement and sensation—is pinched, leading to pain, numbness, or even muscle atrophy in the thumb pad. Venous TOS, affecting roughly 4% to 5% of patients, involves the subclavian vein. This can cause swelling, prominent veins, or 'effort thrombosis' (Paget-Schroetter syndrome), where overhead motions trigger blood clots.

The rarest form is Arterial TOS, occurring in only about 1% of cases. This involves compression of the subclavian artery, which can lead to a cold, pale hand, aneurysms, or even arterial embolisms. While treatment often focuses on physiotherapy and medication to reduce pressure and improve circulation, severe cases involving significant vascular issues or persistent extra ribs may require surgical intervention.
