It is not water vapor: the complex chemistry of the vape aerosol

In short: While many believe vaping is merely inhaling steam, the reality is far more chemically complex. As e-cigarette use among US high schoolers surged by 900 percent between 2011 and 2015, understanding the distinction between tobacco smoke and electronic aerosols has become a critical public health debate.

The fundamental difference between smoking and vaping lies in the mechanism of delivery. Traditional cigarettes burn tobacco, a process that releases thousands of chemicals, including up to 70 known carcinogens, as well as tar and carbon monoxide. In contrast, electronic nicotine delivery systems (ENDS) heat an e-liquid to create an aerosol. While this process avoids many of the toxins found in smoke, it is not harmless. These aerosols can contain additives, flavors, and other toxic substances that pose risks to both users and those exposed to second-hand aerosol.

The debate often centers on harm reduction versus new risks. For adult smokers, switching completely to vaping can reduce exposure to toxins associated with heart disease, lung disease, and stroke. In 2022, tobacco experts noted that in the short and medium-term, vaping poses only a small fraction of the risks associated with smoking. However, the long-term health impacts remain unclear. Furthermore, nicotine itself is highly addictive and can negatively impact brain development in children and adolescents, potentially leading to anxiety or learning disorders.

A significant global concern is the aggressive marketing of these products to youth. With at least 16,000 attractive flavors available, many devices feature sleek designs, cartoon characters, or appearances resembling toys. The World Health Organization notes that 88 countries currently have no minimum purchase age for e-cigarettes, and 74 countries lack any regulation for these products. This has contributed to a trend where e-cigarette use among young people exceeds adult use in several nations, with evidence suggesting these products can increase conventional cigarette uptake among non-smoking youth by nearly three times.
