The invisible progression from mild gum irritation to bone loss
In short: Gingivitis is a common condition that affects the majority of people. However, if left untreated, it can progress into periodontitis—a chronic inflammatory disease that attacks the very foundation of your teeth. Understanding how clinicians distinguish between these two stages is vital for preserving oral health.
Periodontal diseases are characterized as chronic inflammatory conditions affecting the tissues surrounding the teeth. While gingivitis is the mildest form, it serves as a precursor to periodontitis. Research over the last 40 to 50 years has solidified our understanding of these conditions, confirming that bacterial plaque is the primary initiator of gingival inflammation—a fact first demonstrated in experimental studies during the 1960s.
The prevalence of these issues varies significantly across the population. While most people will experience some form of gingivitis, advanced periodontitis is estimated to affect between 5% and 15% of adults. Because both conditions are highly prevalent, clinical guidelines recommend that every patient undergo a periodontal assessment as part of a routine oral examination. This includes initial screenings using standardized tools like the Periodontal Screening Record or the Basic Periodontal Examination.
When periodontitis is identified during screening, a more comprehensive full-mouth assessment is required. Clinicians must record specific data points including probing depths, bleeding patterns, plaque levels, tooth mobility, and recession. They also look for furcation involvement—the space between the roots of teeth. Radiographic assessments are then used to measure alveolar bone destruction based on the specific clinical situation.
Modern diagnosis does not just look at the teeth; it incorporates systemic risk management. A central component of periodontal therapy involves assessing a patient's overall health, specifically looking for factors like smoking status and diabetes. By identifying these risks early, healthcare providers can implement management strategies, such as promoting smoking cessation, to prevent further tissue destruction.
