In short: Mesothelioma is a rare, aggressive cancer of the tissue lining our organs, most commonly caused by asbestos exposure. Because the disease can remain dormant for decades, the link between a brief encounter with fibers and a diagnosis often feels disconnected, masking the true danger of this silent, long-term health threat.

Mesothelioma develops from the mesothelium, the thin tissue layer covering internal organs, most frequently the lining of the lungs. While the disease is rare, it is notoriously aggressive. Its primary driver is asbestos, a mineral fiber that, when inhaled or ingested, triggers chronic inflammation. This process is not immediate; the latency period between initial exposure and the onset of symptoms typically spans 25 to 70 years, peaking at 30 to 40 years. Even brief exposures, lasting as little as one to three months, have been documented to cause the disease.

The mechanism behind this malignancy involves the body's own stress response. Asbestos fibers cause chronic tissue injury, prompting mesothelial cells to release Damage-Associated Molecular Patterns (DAMPs). One key regulator, High-Mobility Group Box 1 (HMGB1), drives a self-sustaining inflammatory circuit that recruits macrophages and promotes tumor growth while simultaneously helping the cancer evade the immune system. Other DAMPs, such as IL-33 and extracellular ATP, further contribute to this environment of chronic inflammation and ineffective antitumor immunity.

Diagnosis is notoriously difficult, as the symptoms—shortness of breath, chest pain, and cough—often mimic other conditions like tuberculous pleurisy. Because asbestos was widely used in construction, insulation, and automotive parts until the late 20th century, many older buildings still harbor risks. Even today, there is no consensus on a 'safe' threshold for asbestos exposure; regulatory bodies like the UK's Health and Safety Executive assume no such level exists. While surgery, radiation, and chemotherapy are standard treatments, the five-year survival rate remains low, averaging 8% in the United States, underscoring the critical need for early detection and continued research into the molecular pathways that drive this disease.
