In short: Optic neuritis occurs when the optic nerve, the vital cable connecting your eye to your brain, becomes inflamed. Often causing pain during eye movement and blurred vision, this condition can be an isolated event or, more significantly, the first warning sign of an underlying autoimmune disorder like multiple sclerosis.

The optic nerve is a bundle of fibers coated in myelin, a substance that ensures electrical impulses travel rapidly from the eye to the brain. In optic neuritis, the immune system mistakenly attacks this protective coating. This inflammation disrupts the transmission of visual data, leading to symptoms that typically include diminished vision, difficulty perceiving vibrant colors, and pain that intensifies when you move your eye. While it most frequently affects one eye, it can occasionally involve both.

The condition is most common in adults aged 20 to 40, particularly white females. While the exact cause is not always clear, it is frequently linked to multiple sclerosis (MS). About 50% of people with MS will experience optic neuritis at some point, and it often serves as the initial clinical presentation of the disease. Other potential triggers include viral infections, autoimmune conditions like lupus, and neuromyelitis optica—a distinct, often more severe disorder that targets both the optic nerve and the spinal cord.

Diagnosis involves a thorough medical evaluation, including visual acuity tests, MRI scans, and sometimes blood tests to identify specific antibodies. The outlook is generally positive for typical cases; about 90% of people recover normal or near-normal vision within six to 12 months. While some patients receive intravenous steroids to reduce inflammation and potentially accelerate recovery, many cases resolve on their own. However, because optic neuritis can signal a progressive neurological condition, medical monitoring is essential to catch and manage any underlying disease early.
