In short: You might be kicking, jerking, or twitching every 20 to 40 seconds while you sleep. Most people with this neurological disorder are completely unaware of the movements, but their bed partners often notice the disruption. It is a hidden rhythm of the nervous system that can impact your long-term health.

Periodic limb movements of sleep (PLMS) is a neurological movement disorder characterized by repetitive, involuntary jerks of the arms or legs. While it primarily affects the lower extremities—specifically the big toe, ankle, knee, and hip—it can also occur in the upper body. These movements typically happen at regular intervals of 20 to 40 seconds. Because they are involuntary, most sufferers are unaware of them, though they may cause frequent awakenings or be noticed by a sleeping partner.

Clinically, there is a distinction between PLMS and Periodic Limb Movement Disorder (PLMD). Healthcare providers diagnose PLMS when these movements occur alongside another condition, such as restless legs syndrome (RLS), which affects 80% to 90% of cases. In contrast, PLMD is diagnosed only when no underlying cause—like sleep apnea or RLS—is found. Research estimates that PLMD and PLMS affect between 4% and 11% of adults and 5% to 8% of children. While the exact cause is unknown, researchers suspect a malfunction in the spinal cord.

The implications of these movements extend beyond a restless night. Studies indicate that people with sleep movement disorders tend to have higher blood pressure, potentially increasing the risk of stroke and heart attack. While there is no specific treatment for PLMS, medications used for RLS are often prescribed. In specific cases where the disorder is accompanied by excessive daytime sleepiness, medications like methylphenidate have shown therapeutic effects in clinical reports. Managing risk factors, such as quitting smoking or increasing physical activity, is currently the primary recommendation for prevention.
