In short: In 1961, a new strain of cholera emerged in Indonesia, triggering a wave of disease that has lasted decades. Recent genomic sequencing reveals that specific mutations in regulatory genes may have provided the biological toolkit necessary for this bacterium to conquer continents.

The seventh cholera pandemic, caused by the Vibrio cholerae El Tor biotype, began in Sulawesi, Indonesia, in 1961. While the pandemic has progressed through three distinct waves, recent genomic analysis of 22 isolates from 1961 to 1979, alongside 152 public genomes, has clarified the mechanics of its early expansion. The first wave actually unfolded in two distinct stages: Stage 1 spread across Asia, while Stage 2 moved into the Middle East and Africa.

The ability of this strain to spread so effectively may be linked to three specific non-synonymous mutations in regulatory genes: csrD, which acts as a global regulator; acfB, involved in chemotaxis; and luxO, which manages quorum sensing. These genetic changes, alongside the acquisition of a novel IncA/C plasmid in certain lineages, likely contributed to its pandemicity. This evolutionary shift was part of a broader biotype transition from the Classical to the El Tor biotype.

The pandemic's footprint has been massive and varied. By 1966, it had reached most of Asia, and by 1971, an upsurge brought it to Europe and Africa. In 1991, a major resurgence saw 570,000 cases, including an epidemic in Peru that killed roughly 10,000 people. While modern medicine has reduced mortality rates from 50% to less than 3% by the 1990s, the disease remains a persistent threat in regions with poor sanitation.
