In short: In the 2014 psychological thriller 1: Nenokkadine, Gautham is a musician living a fractured existence. While battling hallucinations that obscure the distinction between imaginary foes and actual killers, the protagonist searches for his parents whilst dealing with schizophrenia and a 25 percent loss of grey matter.

Directed by Sukumar, the film follows Gautham, a rock musician struggling with profound neurological and psychological challenges. He suffers from schizophrenia and is missing 25 percent of his brain's grey matter. This condition leads him to hallucinate a mysterious figure he believes is responsible for the murder of his parents, whom he believed were dead since childhood. His delusions are exacerbated by a traumatic childhood where he was convinced he was an orphan. This mental state causes him to mistake an aged taxi driver for one of his imagined enemies.

The narrative complexity and length initially led to mixed reviews and a box office failure upon its release on 10 January 2014. However, the film has since gained a cult following and was recognized by Film Companion as one of the "25 Greatest Telugu Films of the Decade." The plot eventually reveals a conspiracy involving the distribution of "Golden rice," a weather-resistant variety that Gautham's father refused to sell for free, leading to his murder by business associates Antonio Rosarius and an unnamed NRI in London. The film's visual style was crafted by cinematographer R. Rathnavelu, who utilised various cameras including Red Epic and underwater equipment. The story's resolution involves a mysterious bag containing a Rubik's Cube, a key, and a British coin.

With a budget ranging from ₹60–70 crore, this production stood as the most costly Telugu movie when it debuted. Mahesh Babu prepared for the part by completing a strict 12-week Dynamic Transformation Plan (DTP) led by Kris Gethin, then transitioned to Neil Hill's Yoda Three Training regime (Y3T) to gain the fitness needed for the film's heavy action. Mahesh Babu's physical transformation was supported by stylist Ashwin Malwe to ensure a sophisticated appearance. Additionally, the director chose to have the lead actor wear glasses to project a more perfectionistic persona.
