During his rookie season, he finished third in the American League Rookie of the Year voting.
The most defining moment of his career occurred in early 2020. Mancini underwent surgery to remove a malignant tumor from his colon and was diagnosed with stage 3 colon cancer. He spent the entire 2020 season on the 60-day injured list undergoing chemotherapy. His return in 2021 was met with a standing ovation from the Baltimore crowd, and he was subsequently honored with the AL Comeback Player of the Year Award.
Mancini's later career saw him transition between several organizations, including the Houston Astros, Chicago Cubs, and Arizona Diamondbacks. In 2022, he played a pivotal role for the Astros, helping them clinch the AL West division title and ultimately winning his first World Series title. He made a memorable defensive play in Game 5 of that series, snagging a hard-hit ball to preserve a 3–2 lead. He officially announced his retirement on August 18, 2026, returning to the Orioles for a final one-day contract.
