A brilliant military surgeon lived a double life that only surfaced after his death.
In short: Dr. James Barry rose to the rank of Inspector General of Hospitals, the second-highest medical position in the British Army. Yet, his true identity as a woman born Margaret Anne Bulkley remained hidden from colleagues and the public for over five decades, only becoming known after he passed away.
Born in Cork around 1789, Barry navigated a society that barred women from medical education. To pursue his ambition, he adopted a male identity, a transition supported by a circle of influential friends including General Francisco de Miranda. By 1809, he had arrived in Edinburgh to study medicine, eventually qualifying as a doctor in 1812. His career was marked by significant medical achievements, including performing the first successful caesarean section by a European in Africa, and a fierce commitment to the welfare of soldiers.
Barry’s military service spanned the British Empire, from South Africa to Canada. His records reveal a man of intense conviction, often clashing with authority when he felt the needs of his patients were being ignored. In 1859, he famously threatened to expose church officials in the London Times after they refused to adjust service times for soldiers on sanitary grounds. He also served during the Crimean War, where he reportedly engaged in disputes with Florence Nightingale, reflecting his uncompromising nature regarding hospital standards.
The revelation of Barry’s anatomy after his 1865 death sparked enduring historical debate. While some early accounts speculated about his past, modern research into his personal army service records and correspondence—such as a 1809 letter to his solicitor—has confirmed his identity as Margaret Anne Bulkley. Today, he is recognized as a pioneering transgender figure whose professional excellence forced his contemporaries to respect a man whose private history defied the rigid gender expectations of the 19th century.
