The biological limits of our liver's remarkable ability to regrow
In short: Humans can donate over half of their liver, and the remaining tissue will grow back within a year. This incredible regenerative power suggests we could theoretically become perpetual donors—but medical reality is far more complex than simple regrowth.
Living liver donation involves removing a portion of a healthy person's liver to save someone with end-stage liver disease, primary liver cancer, or metabolic diseases affecting children. Because of the liver's unique anatomy, surgeons can remove specific lobes: typically the larger right lobe from adults and the smaller left lateral lobe from children. This procedure has been successfully performed in the UK since 1995 to address the shortage of organs donated by deceased individuals.
While the organ is capable of significant regeneration, it is not an infinite resource. The process involves high-stakes medical risks; for example, the risk of a donor dying during a right lobe transplant is approximately 1 in 200, while the risk for a child's left lobe transplant is about 1 in 1,000. These figures underscore that while regrowth occurs, it is not a trivial biological feat and carries inherent dangers to the donor's health.
The demand for these organs remains high, with around 650 people in the UK currently waiting for transplants. While many donors are close relatives or friends, 'non-directed altruistic donors' also volunteer to help strangers. However, because of the physiological stress and specific surgical risks involved, medical teams require rigorous testing to ensure a donor is suitable. The ability to regrow tissue does not mean a person can be repeatedly harvested without significant health consequences.
Ultimately, living donation is a voluntary act intended for life-saving intervention rather than a cycle of repeated extraction. While the liver's regenerative capacity is one of the body's most impressive feats, it serves as a mechanism for recovery and survival in critical illness, not a license for indefinite donation.
