When your lungs lose their stretch, they begin to overfill

In short: Imagine your lungs losing their ability to snap back like a rubber band. In emphysema, the tiny air sacs that facilitate breathing are destroyed, merging into large pouches that trap air inside. This structural collapse creates a paradox: as the disease progresses, the lungs become physically overfilled while oxygen fails to reach the blood.

Emphysema is a primary component of Chronic Obstructive Pulmonary Disease (COPD), categorized as a chronic inflammatory lung disease. The core pathophysiology involves the destruction of alveolar walls—the microscopic structures where gas exchange occurs. When these walls rupture, the distal air spaces beyond the terminal bronchioles enlarge gradually. This structural decay leads to a significant loss of lung elasticity and impaired gas exchange, making it increasingly difficult for oxygen to enter systemic circulation.

The mechanical failure of the lungs is often described as the formation of one "big pouch" from many smaller ones. Because the lungs lose their elastic recoil, air becomes trapped within these enlarged spaces rather than being exhaled efficiently. While early symptoms may be mild or overlooked, the progression leads to chronic cough with mucus production, wheezing, chest tightness, and significant shortness of breath. Systemic impacts are also common, including fatigue, weight loss, anxiety, depression, sleep deprivation, and cardiovascular problems.

While cigarette smoking is identified as the most frequent etiologic factor for emphysema, environmental pollution and respiratory infections also contribute to its pathogenesis. The disease is considered an "epidemic" with a high economic burden, currently affecting approximately 9% to 10% of the global population. In future years, this condition is anticipated to be the third leading reason for death. Because symptoms can mimic other respiratory conditions, accurate diagnosis is essential for effective management and to prevent exacerbation by infections.

Current research focuses on understanding these pathogenic mechanisms to develop evidence-based and personalized medicine. This includes exploring experimental models that mimic human disease features to test available therapeutics and exploratory approaches aimed at improving patient health outcomes.
