Why the recent monkeypox outbreak isn't as deadly as feared

In short: While the headlines often spark panic, the data suggests a different reality. Recent global outbreaks have shown a significantly lower mortality rate than historical records predicted, forcing scientists to rethink how the virus behaves in different settings and populations.

Mpox, formerly known as monkeypox, is an infectious disease caused by a virus in the Orthopoxvirus genus. It is characterized by a painful rash, fever, and swollen lymph nodes. The virus exists in two distinct clades: Clade 1 (found primarily in central and east Africa) and Clade 2 (found at low levels globally). A major global outbreak began in 2022, primarily driven by the Clade IIb strain, while more recent concerns have shifted toward Clade Ia and Ib strains in the Democratic Republic of the Congo and other regions.

The reason for the 'cautious sigh of relief' among researchers lies in the mortality statistics. During the 2022 outbreak, the death rate was approximately 0.04%, based on at least 22 deaths out of more than 57,000 confirmed cases. This figure is remarkably lower than the 1–3% death rate observed in past outbreaks caused by similar strains in West Africa. This discrepancy has forced scientists to re-evaluate their assumptions about the severity of the disease, as the symptoms of severe disease in recent cases differ from those seen in historical data.

Transmission occurs mainly through close physical contact, such as skin-to-skin contact, kissing, or sharing contaminated items like bedding and towels. It can also spread through respiratory particles when individuals are face-to-face. While most people recover fully with supportive care—focusing on hydration, nutrition, and pain management—the virus remains a public health priority. Vaccination and public health interventions are recommended to manage the ongoing threat, especially as Clade Ib has been detected beyond Africa as of August 2024.
