The biological mechanics of the most intense physical feat in mammals

In short: Labor is often described as the Mount Everest of physical functions. It is a complex, multi-stage orchestration of muscular contractions, hormonal shifts, and anatomical transformations designed to move a baby from the uterus to the outside world.

The process begins with the first stage of labor, characterized by the transformation of the cervix. This involves both effacement—the thinning of the cervical tissue, often compared to the way a mint thins as it is sucked—and dilation, the widening of the cervical opening. While early labor can begin with the cervix dilated anywhere from 0 to 3 centimeters, the stage concludes only when the cervix reaches full dilation of approximately 10 centimeters. This stage is divided into phases: early labor, which can last 6 to 12 hours, and active labor, which typically lasts 4 to 8 hours. For first-time mothers, the duration from the start of established labor to full dilation is usually between 8 and 18 hours, though subsequent births may be faster, lasting 5 to 12 hours.

The second stage involves the descent and birth of the baby. As the fetus moves down the birth canal, its position is measured in stations. At station 0, the baby is aligned with the ischial spines of the pelvis; as it descends through stations +1, +2, and +3, crowning begins. During this phase, contractions typically occur every two to five minutes and last between 60 and 90 seconds. This stage can last anywhere from thirty minutes to several hours.

The third stage is the delivery of the placenta, which generally occurs 5 to 25 minutes after the birth. This process is aided by the hormone oxytocin, which promotes continued uterine contractions. Finally, the fourth stage is the postpartum period, lasting until 6 weeks after birth, a time focused on newborn care and bonding. While labor durations vary wildly—from a rare 1 hour to approximately 24 hours for a first-time mother—each experience is unique.
