In short: For runners, dancers, and military trainees, the onset of a new fitness regimen can bring a sharp, stabbing pain along the tibia. This common condition, known medically as medial tibial stress syndrome, occurs when the repetitive stress of high-impact activity outpaces the body's ability to repair itself.

Shin splints are characterized by pain, tenderness, or swelling along the inner side of the shinbone, or tibia. While the sensation often begins as a mild ache that vanishes once exercise stops, it can progress into a persistent, ongoing pain. In some cases, the discomfort may feel like a sharp, stabbing sensation.

The underlying mechanism involves inflammation in the tissues that connect muscles to the bone. When you increase the duration, frequency, or intensity of your workouts too rapidly, these connective tissues and the bone itself are overloaded. This is particularly common when exercising on hard surfaces like concrete or uneven ground, such as hills.

Preventing recurrence often requires a gradual approach. Experts suggest a progression of about a 10% increase in high-impact activity per week. Additionally, strengthening the legs, ankles, hips, and core can help prepare the body for high-impact sports. Using well-cushioned shoes—replaced every 350 to 500 miles—and arch supports can also mitigate risk, especially for those with flat feet or high arches.

While usually not serious and often resolving within a few weeks of rest, ignoring the pain can lead to complications. Pushing through the discomfort increases the risk of stress fractures, which are tiny cracks in the bone caused by overuse. Women may be more susceptible to progressing from shin splints to stress fractures, partly due to higher incidences of diminished bone density.
