A dropped wrench triggered a catastrophic nuclear missile explosion in rural Arkansas
In short: In 1980, a routine maintenance check at a Titan II missile silo spiraled into a disaster. A single socket fell eighty feet, piercing a fuel tank and igniting a hypergolic fire. The resulting blast ejected a nine-megaton thermonuclear warhead, narrowly avoiding a catastrophic nuclear detonation in the American heartland.
The incident occurred on September 18, 1980, at Launch Complex 374-7 near Damascus, Arkansas. While performing pressure checks on the oxidizer tank, a technician accidentally dropped an eight-pound socket from a heavy ratchet. The tool plummeted roughly eighty feet, striking a thrust mount before puncturing the missile's skin. This breach released Aerozine 50 fuel, which spontaneously ignites upon contact with the missile's dinitrogen tetroxide oxidizer.
As the fuel leaked for nearly six hours, the situation grew dire. Military personnel attempted to enter the silo to manage the volatile atmosphere. Two airmen, John Devlin and Rex Hukle, were forced to use bolt cutters and crowbars to breach the facility's security doors after the command team evacuated. Later, David Lee Livingston and Jeff Kennedy were sent to activate an exhaust fan. At approximately 3:00 a.m. on September 19, the fuel ignited, likely triggered by electrical arcing in the fan.
The explosion was immense, catapulting a 743-ton silo door into the air and ejecting the second stage along with its W-53 warhead. While Livingston tragically died from his injuries and twenty-one others were harmed, the warhead's safety mechanisms prevented a nuclear detonation or radioactive leakage. President Jimmy Carter and Arkansas Governor Bill Clinton publicly reassured citizens that the situation was contained. The site was never repaired and was eventually buried, later earning a listing on the National Register of Historic Places in 2000.
