In short: In 1998, Garrison Hearst was an unstoppable force, setting franchise records for the San Francisco 49ers. But a gruesome ankle injury threatened to end his career forever, leading to a rare bone condition that doctors feared would prevent him from ever playing again.

During a 1998 playoff game against the Atlanta Falcons, Hearst suffered a severe ankle break after his foot became caught in the Georgia Dome turf. The injury led to avascular necrosis, a condition where circulatory problems choke off the blood supply, causing the talus bone in his foot to die. This condition is famously associated with Bo Jackson, whose hip suffered the same fate, forcing his retirement.

Despite the grim prognosis, Hearst underwent over two years of rehabilitation. In 2001, he achieved a historic milestone by becoming the first player in NFL history to return to professional football after suffering from avascular necrosis. His comeback was highly successful; he rushed for 1,206 yards and earned the NFL Comeback Player of the Year award.

Hearst's impact on the field was quantifiable. While the 49ers had a 10–22 record in the two seasons prior to his return, they went 12–4 in 2001 with him active. Before his injury, Hearst had established himself as a premier talent, setting a then-franchise record of 1,570 rushing yards in a single season and recording a 96-yard game-winning touchdown run that was later featured by NFL Films as one of the greatest in history.

Beyond his physical recovery, Hearst's career was marked by significant collegiate achievements. Playing for the Georgia Bulldogs from 1990 to 1992, he was a unanimous All-American and the 1992 SEC Player of the Year. He finished his college career trailing only Herschel Walker in rushing yardage, totaling 3,232 yards.
