In short: The Sovereign Military Order of Malta is a Catholic religious order that functions like a country. Despite lacking national territory, it maintains diplomatic relations with 115 states, issues its own passports, and holds United Nations observer status. It is a unique remnant of history that continues to operate globally today.

Tracing its roots to a hospital founded in Jerusalem around 1048, the order evolved into a military and chivalric force during the Crusades. Officially recognized by a papal bull in 1113, it gained independence from local secular and religious authorities. Over centuries, the knights held territory in Cyprus, Rhodes, and eventually Malta, which they famously defended against Ottoman forces in 1565. However, Napoleon’s 1798 invasion of Malta forced the knights into exile, leading to a long period of territorial displacement.

Today, the order is headquartered in Rome, where its two primary buildings enjoy extraterritorial status. While it no longer governs a nation, its international legal status remains intact, allowing it to sign treaties and maintain embassies. The order is led by an elected Grand Master and operates with a government structure similar to that of a modern state. Its membership includes about 13,500 Knights, Dames, and Chaplains, some of whom are professed religious members bound by vows of poverty, chastity, and obedience.

The order’s modern mission is purely humanitarian. It employs roughly 52,000 medical professionals and relies on 100,000 volunteers to provide aid in over 120 countries. Through Malteser International, it responds to natural disasters, epidemics, and war, assisting the vulnerable regardless of ethnicity or religion. While the order maintains its ancient traditions and heraldry, its contemporary relevance is defined by this global relief work, which has expanded significantly since the 20th century.

Despite its long history, the order is not without internal friction. A notable leadership crisis occurred in 2016 following a dispute over charitable projects, leading to the intervention of Pope Francis. This event prompted debates regarding the order's independence and its relationship with the Holy See. Nevertheless, the order continues to protect its heritage and emblems, having registered sixteen versions of its identity in approximately one hundred countries to prevent fraud.
