A common blood thinner could be a triple-threat weapon against COVID-19

Finally, a Drug That Helps With the Worst COVID-19 Infections · 6:45 · Video: SciShow · Watch at the source

In short: Inhaled heparin is emerging as a versatile defense against severe respiratory infections. While typically used as an injection for blood clots, this affordable drug offers a unique three-in-one mechanism that could protect the lungs from various viruses and bacteria, including pneumonia and influenza.

An international study led by the Australian National University (ANU) and King's College London analyzed data from nearly 500 hospitalized COVID-19 patients across six countries. Significant outcomes occurred: individuals given inhaled heparin experienced a 50% reduction in the need for mechanical ventilation plus a marked decrease in mortality rates versus those on conventional treatment. What makes heparin unique is its "triple-action" profile. Unlike many treatments that target a single pathway, heparin acts as an anticoagulant, an anti-inflammatory agent, and a pan-antiviral. Professor Clive Page of King's College notes that no other drug currently possesses this specific combination of properties.

By targeting the lungs directly through inhalation, the drug aims to prevent infections from damaging lung tissue and spreading. Beyond COVID-19, researchers believe inhaled heparin could serve as a broad-spectrum tool for other serious respiratory infections, such as pneumonia, influenza, and RSV. This is particularly promising for immunocompromised individuals, including cancer patients. Because the drug is already widely available and inexpensive, it offers a scalable solution for low-income countries. Researchers are currently developing an improved formulation specifically optimized for inhalation and plan to conduct further trials in Europe to confirm its effectiveness against other common respiratory pathogens.

The study's findings were published in eClinicalMedicine and presented at the European Respiratory Society (ERS) International Congress in late 2022. While the results are promising, the researchers emphasize that further development is necessary before inhaled heparin can be routinely adopted as a standard clinical treatment.
