The man who caught smallpox became a shield against polio
In short: In 1977, Ali Maow Maalin became the last person on Earth to contract naturally occurring smallpox. Instead of retreating from his experience with disease, he spent the next three decades in some of the world's most dangerous regions, using his personal history to convince others to take life-saving vaccines.
Ali Maow Maalin was a hospital cook near Mogadishu who contracted smallpox in October 1977. While transporting infected children to an isolation camp, he was exposed to the virus for as little as 5 to 15 minutes. Despite his infection, he successfully avoided spreading the disease to others, and three years later, Somalia—and the rest of the world—was declared free of smallpox. His experience was unique; while some records suggest he may have received a vaccine that failed, Maalin himself stated in 2007 that he simply avoided the shot because he feared it would hurt.
Maalin transformed his role from victim to advocate. In 2004, he became an official polio vaccinator in Somalia. His work was fraught with physical danger; during a period of civil unrest, several vaccinators were shot by militia groups who distrusted the safety of the medicine. Maalin used a specific psychological tactic to overcome this: he shared his own story of smallpox to build trust with parents and leaders. By framing his past as a cautionary tale of 'doing something foolish,' he helped stamp out Somalia's polio outbreak in 2005.
Even after Somalia was declared polio-free in 2007, Maalin returned to the field when the virus re-emerged in 2013. He continued organizing immunization campaigns in turbulent regions where militia groups controlled access. He died on July 22, 2013, at age 59 from complicated malaria while actively monitoring a campaign. His life serves as a bridge between two of humanity's greatest medical achievements: the total eradication of smallpox and the ongoing effort to eliminate polio.
