In short: Behind the facade of a charitable NGO lies a dark web cult. To synthesize a potent drug for the global elite, they harvest endocrine glands from terrified victims. This high-stakes neo-noir thriller reveals how a ruthless officer must infiltrate a syndicate that demands filmed murders for entry.

The film follows SP Arjun Sarkaar IPS, a formidable officer of the Homicide Intervention Team (HIT). The narrative unfolds through a series of flashbacks while Arjun is incarcerated. He reveals that he deliberately engineered his own arrest to infiltrate a sinister organization known as 'CTK' (Capture, Torture, Kill). To gain entry into this dark web cult, prospective members are required to upload two filmed murders to the syndicate. The investigation leads Arjun to a secluded Burmese palace in Naharlagun, Arunachal Pradesh. In this scene, he uncovers the secret activities of the group: they posed as a non-profit organisation called 'Keen to Care' (KTC). Their objective was to harvest endocrine glands from victims to supply a global pharmaceutical trade.

This trade aimed to synthesize a highly potent version of sildenafil for the global elite. To maintain secrecy, the syndicate used decoys and forced hostages into horrific executions. The story concludes with a revelation that Arjun's fellow inmate, Samuel Joseph, was the mastermind behind KTC. Arjun eventually executes him inside his prison cell. The film, which premiered on May 1, 2025, received mixed critical reception, with reviewers praising actor Nani's intense performance while criticizing the narrative for prioritizing gore and brute force over emotional depth. Despite the mixed reviews, it became the eighth highest-grossing Telugu film of the year, grossing ₹88 crore worldwide within its first six days.

The production of the film was marked by significant logistical shifts. First revealed in 2022, the production was held up by the making of 'They Call Him OG.' Main filming occurred between September 2024 and March 2025 across various sites like Hyderabad, Visakhapatnam, Jaipur, Jammu & Kashmir, and Arunachal Pradesh. Because of its intense brutality, the movie received an 'A' rating, which required some blurring and trimming to control the amount of blood.
