In short: While the Pope is the global leader of the Catholic Church, he is also the Bishop of Rome. This dual role governs a diocese that stretches across both Italy and Vatican City, managed through a unique structure of vicars, sectors, and auxiliary bishops that has recently undergone significant centralization.

The Diocese of Rome is a unique ecclesiastical entity that serves as the primatial see of Italy. Geographically, it encompasses 881 square kilometres, including the entirety of Vatican City and a large portion of the Italian municipality of Rome. Because the papacy is a sovereign entity, the diocese operates across two distinct jurisdictions: the Vicariate of Vatican City and the Vicariate of Rome. The former is primarily focused on serving pilgrims and tourists, while the latter manages the pastoral needs of the local population through 336 active parishes.

The governance of this territory has evolved significantly in recent years. Historically, the vicar general held substantial administrative power, acting on behalf of the Pope. However, in January 2023, Pope Francis issued the apostolic constitution In Ecclesiarum Communione, which restricted the vicar general's autonomy. The Pope now requires that no important initiatives or extraordinary administrative actions be undertaken without his direct approval. This shift toward a more collegial model was further refined by a pontifical decree in February 2026, which reorganized internal territorial boundaries and management sectors.

The diocese is divided into five pastoral sectors—north, south, east, west, and center—each overseen by an auxiliary bishop. These bishops collaborate with the vicar general and the vicegerent to manage the diocese's complex demographics, which include over 1,200 diocesan priests and thousands of religious personnel. While the Archbasilica of Saint John Lateran serves as the official cathedral, the diocese's reach is so vast that parts of the city of Rome actually fall under the jurisdiction of neighboring dioceses like Ostia and Porto-Santa Rufina. This structure ensures that the Pope’s local ministry remains distinct from his universal role as the Supreme Pontiff.
