In short: Eluru, a city with roots stretching back to the second century, is a vital commercial center in Andhra Pradesh. Yet, in 2020, it captured national attention for a different reason: a sudden, mysterious illness that struck hundreds, forcing a multidisciplinary investigation into the city's water and environmental health.

Eluru, historically known as Helapuri, has served as a seat of power for numerous dynasties, including the Salankayanas, Vishnukundinas, and Eastern Chalukyas. Positioned strategically on the banks of the Tammileru River, the city evolved from an ancient capital into a colonial-era administrative center within the Madras Presidency. Its modern identity is defined by its role as a key trade hub for rice, tobacco, and its renowned pile carpets, as well as its proximity to Kolleru Lake, one of India’s largest freshwater wetlands.

In December 2020, the city faced a public health crisis when hundreds of residents suddenly suffered from seizures and loss of consciousness. The outbreak prompted an urgent response from teams including the World Health Organization and AIIMS, Mangalagiri. Investigators analyzed blood, urine, and water samples, eventually identifying the presence of Triazofos, an organophosphate pesticide, in household drinking water. While the municipal supply was clear, the contamination was found at an average concentration of 1.00 μg/L in domestic sources, highlighting the complexities of urban water management.

Beyond the immediate health response, the incident drew focus to the broader environmental degradation of the region. Kolleru Lake, once a thriving habitat for millions of migratory birds like the grey pelican, has seen its avian population dwindle due to pollution and habitat loss. Today, Eluru continues to balance its historical legacy and economic growth with the modern challenges of environmental sustainability, as seen in its ongoing efforts to monitor air quality and manage solid waste through the Eluru Urban Development Authority.
